DR MARTIN SCURR: Is severe tiredness ‘chronic fatigue’ or something else?

My daughter, now 40, had a traumatic birth six years ago — soon after she started feeling unnaturally tired . A private consultant diagnosed fibromyalgia, and she has ME. She has tried everything — acupuncture, counselling, reiki — but the fatigue is crippling.

I understand your concern, as your daughter has been unwell for six years. My feeling is that there is an unrecognised diagnosis in the background.

Fibromyalgia typically causes fatigue, poor sleep patterns, problems with memory and persistent chronic pain — the condition is the most common cause of widespread musculoskeletal pain.

The closest thing we have as a test for it is a physical examination to check for 18 specific points of tenderness in soft tissues.

In some patients, their symptoms of fibromyalgia are very similar to those of myalgic encephalomyelitis (ME), also called chronic fatigue syndrome (CFS).

Like fibromyalgia, ME/CFS is an illness of unknown cause also characterised by fatigue.

In the absence of a test, doctors diagnose it by relying on experience and skill — though I’m afraid too often ME/CFS is given as a fallback diagnosis for those with long-term severe fatigue. Because of the overlap of symptoms it’s not unusual for people to be diagnosed with both conditions.

My thought is that in your daughter’s case, the issue may be either post-traumatic stress disorder (PTSD), or protracted postnatal depression.

PTSD can be triggered by a single terrifying event — there is no doubt from your longer letter that this happened to your daughter in childbirth. Similarly, postnatal depression is a common but misunderstood and frequently ignored condition that can last for years, leading to general malaise, such as you describe in your longer letter.

I urge you to help your daughter see a consultant psychiatrist, who can assess her on the basis of a detailed appraisal of her history and symptoms.

Suitable treatment can then begin and there is reason for optimism. The first step is to ask her GP for a psychiatric referral.

My wife, 82, has had backache for 45 years. It became worse a few years ago, causing her to give up activities she enjoyed such as bowling. As well as degeneration of the lower spine, she suffers from arthritis: her quality of life is poor, but the GP’s only solution is co-codamol, which makes her drowsy. She’s also living with Alzheimer’s.

It is a heart-breaking predicament for you. Managing dementia is in itself a major burden, but the overriding, immediate issue is pain control.

I wonder whether as well as the back pain and arthritis, there is a third element in your wife’s case — possibly nerve entrapment due to the arthritis or collapse of one of the vertebrae, which would cause symptoms you describe in your longer letter, including shooting pain around the back of both thighs.

I suggest you and your wife talk to her GP and ask about referral to a pain management clinic.

(Surgery to relieve nerve entrapment isn’t out of the question in a patient with early dementia — indeed many of your wife’s symptoms may be far easier to deal with if she’s not in chronic pain.)

In the interim, a modest dose of an anti-inflammatory such as diclofenac, taken regularly, with paracetamol now and then, might be a good choice. And ask your GP about a house visit from an occupational therapist, possibly with a physiotherapist.

In the world of medical diagnosis we sometimes use the phrase incidentaloma, a glib non-scientific term for finding an unexpected abnormality when carrying out tests for some other reason: we’re checking for one thing, only to find another.

I know of someone who recently had a health MOT, which included a colonoscopy, an examination of the bowel guided by a scan: this revealed cysts in her pancreas of a type likely to become cancerous. Prompt surgery avoided this.

Yet the value of health MOT screening tests such as these is controversial, not least if people have them done privately, and then arrive at their NHS GP with an abnormality that may not be a problem, i.e. they have no symptoms.

There are other concerns: new blood tests to detect tumour markers have proved disappointing, with false positives and false negatives — and yet you can get these as part of private health MOTs.

I would recommend evidence-based national screening programmes: those for cervical and breast cancers, and stool tests for colon cancer. And rather than private health MOTs, focus on prevention: maintain a healthy weight, eat healthily and exercise. There’s so much more to be gained.

Netflix users discover ‘secret menu’ that has hundreds of hidden movies and shows – here’s how to unlock them now

If you are sick of flicking through the same shows and movies on Netflix, there’s a workaround.

The streaming giant has a secret menu containing hundreds of hidden titles not on the main platform.

The alternative catalog can be accessed using codes associated with a specific genre, such as High Brow Horror, Irreverent TV Comedies and 90-minute movies

There are two ways to access the secret collections. You can do it from any desktop by typing ‘netflix.com/browse/genre/’ into the URL and adding a unique ID code.

Alternatively, if you use Netflix on a TV, you can simply type in the code to the ‘search’ bar. It does not work on mobile or tablets.

The codes are numbers that represent categories that are tucked in the Netflix library and link to content that was offered in the company’s early days.

On the main site or app, Netflix uses an algorithm that customizes each subscriber’s homepage experience by collecting user interests and viewing history data.

It comes as Netflix makes headlines worldwide for all the wrong reasons.

The struggling site announced it was cracking down on password sharing this year, despite using the feature as a promotional tool years prior. A full ban is expected to happen this month in the US.

The company shared the news in a letter to shareholders, noting that ‘widespread account sharing’ has exceeded over 100 million viewers.

These individuals, however, will be able to transfer their profiles to a new paid account, allowing them to carry over their preferences.

The sharing ban comes in response to its first subscriber loss in more than a decade – its customer base fell by 200,000 subscribers in the first quarter of 2023.

And the famously ad-free website introduced a cheaper tier that includes commercials in November 2022, the $6.99 a month ‘Basic with Ads’ option, which differs from the standard $9.99 account.

Advertisements are either 15 or 30 seconds long and play before and during shows and movies.

And leading up to the recent ban on password sharing, the streaming service revealed plans for a paid sharing option.

This will be for multiple users on the same account.

Under the new rules, people watching Netflix using someone else’s account must create their own logins and pay for their own access.

To enable people to share an account, ‘paid sharing’ will be added, allowing multiple users but costing less than a full subscription.

Accounts already come with five ‘profiles,’ but these are solely for use by members of the same household.

‘Every account comes with five profiles that can be used by any member of the household,’ a Netflix spokesperson said.

The new option comes if people wish to share their Netflix account with friends and family living elsewhere.

‘They were hiding in the closet’: Teacher’s daughter says children hid in classroom as Nashville shooting started

The daughter of one of the teachers at the private school where three nine-year-old children and three staff members were fatally shot Monday said her mother hid out in a closet with her students.

Six people – including three children – are dead after transgender female shooter Audrey Hale opened fire at Covenant Presbyterian School in Nashville.

Avery Myrick, whose mother has been a pre-K teacher there for over a decade, said she was unsure if her mother was safe.

Myrick eventually received the call that she had gotten out alive and detailed how she and her students were hiding out from the 28-year-old shooter Hale.

‘She said they were hiding in the closet, and there was shooting all over,’ Myrick said. ‘[She said] they had potentially tried to get into her room.’

Myrick told WSMV that she was heartbroken for people who hadn’t gotten the same call she did to tell her that her mother was safe.

At around 10.13am, Hale opened fire at The Covenant School, shooting and killing nine-year-olds Evelyn Dieckhaus, Hallie Scruggs and William Kinney.

Scruggs was the daughter of Chad Scruggs, the pastor at the affiliated presbyterian church.

Substitute teacher Cynthia Peak, 61, head of school Dr. Katherine Koonce, 60, and custodian Mike Hill, 61, were also killed.

It’s unclear what Hale’s motive was, but police believe she attended the school at some point. She had detailed, drawn maps of the school and had been surveilling it, according to police.

‘We have a manifesto, we have some writings. W e have a map drawn out of how this was all going to take place,’ Nashville Police John Chief Drake said.

The small school is run by a church and does not employ a school resource officer.

Shortly before 10.13am, the woman entered the school by shooting through a locked side door and began opening fire on the second floor.

Police arrived at the scene and heard the gunshots coming from the 2nd floor.

By 10.27am, she had been shot dead. She was armed with two assault-style rifles and a handgun.

Parents have been told to collect their children from a nearby church, but no other information has been given.

‘I know this is probably the worst day of everyone’s lives.

‘I can’t tell you how sympathetic we are,’ a Metro Police officer was heard telling the parents as they waited at a nearby church for updates.

Neighbors of the school watched as parents frantically rushed to the scene to find their children.

‘You don’t send you kid to school thinking there would be an active shooting, especially in this area.

‘Just hearing as a parent myself it’s truly overwhelming. Seeing all the parents run up the hill to see if their children are okay,’ Lisa Debusk told DailyMail.com.

Susan Perkins, 68, told DailyMail.com that her daughter and step-grandson were in the school at the time of the shooting, and unhurt.

Her daughter is a communications director and her step-grandson is a sixth grade student.

‘I talked to my daughter and she was so busy she just couldn’t talk, just enough to say she was okay,’ said the mother, who lives in Knoxville, TN.

‘I’m just shaking with fear and so sorry that this happened,’ she said. ‘I just can’t hardly believe that three children got killed.’

She described Covenant School as a small, tightknit, Christian school where everyone knows and supports each other.

‘I did not think it would ever hap to my school and I would like to see the guns off the street,’ Perkins said.

Alison Grippo, 40, told DailyMail.com that she has three children in Covenant School who survived.

As word spread of the mass shooting, she like other parents were directed to nearby Woodmont Baptist Church, set up as a meeting point as they anxiously waited for their kids.

‘She said her own children, like many others, were still speaking with investigators.

‘We’re waiting to be reunited with our kids,’ Grippo told DailyMail.com. ‘This is just hard to understand or put into words. We praise God in all situations, good and bad.’

There are around 33 staff members in the school and some 210 students.

Many of those students were seen running from the building today in their school uniforms after the shooting began.

Other schools in the area have gone into lockdown as a precaution, but there is not thought to be any additional threat.

Parents are now gathering at a church next to the school.

Tim Dunavant, a pastor at Harstville First United Methodist Church, said he hired Hill to work at the school over a decade ago.

‘He was the last employee that I hired when I ran the kitchen at the Covenant church and school,’ said Dunavant.

‘That was over 13 years ago. He was still working there today when he was shot and killed.

‘I don’t know the details yet. But I have a feeling, when it all comes out, Mike’s sacrifice saved lives.

‘I have nothing factual to base that upon. I just know what kind of guy he was. And I know he’s the kind of guy that would do that.’

He added: ‘Goodbye Mike, I’m going to miss those encouraging texts out of the blue from you.’

Tennessee Gov. Bill Lee tweeted: ‘I am closely monitoring the tragic situation at Covenant…as we continue to respond, please join us in praying for the school, congregation & Nashville community.’

At a press briefing, White House Press Secretary Karine Jean-Pierre said the shooting was proof the ongoing need for stricter gun reform.

First Lady Jill Biden, speaking at the National League of Cities conference on Monday, told the crowd: ‘We just learned about another shooting in Tennessee – a school shooting.

‘I am truly without words. Our children deserve better. We stand with Nashville in prayer.’

Monday’s shooting is the 129th shooting this year.

American actor Harvey Keitel, 83, made his film debut in 1968 in Who’s That Knocking at My Door?, the first feature film directed by famous director Martin Scorsese.

The New York star went on to star in a range of films throughout his career, including Mean Streets, Taxi Driver, and Bugsy, for which he received an Academy Award nomination for his performance as Mickey Cohen in 1991.

He has also worked with Quentin Tarantino on his film about a corrupt copper drowning in gambling debt in Bad Lieutenant in 1992.

He later featured in another of Tarantino’s films, the iconic Pulp Fiction in 1994.

Some of his most recent works includes Wes Anderson’s The Grand Budapest Hotel in 2012 and the Irishman in 2019 where he was reunited with Scorsese and featured alongside Robert De Niro.

In 2021, Mr Keitel worked on Lansky, a biopic about the gangster Meyer Lansky.

The actor has captured the hearts of many fans over the years, who were more than happy to comment on how well the actor is looking at 83. One person said: ‘Great actor, great man. He looks fabulous’.

Another added: ‘Harvey Keitel still looks good’.

A third person commented: ‘He looks fantastic.’

Chevy Chase

Chevy Chase, 79, is an American comedian, actor and writer who became a well-known name as a result of his Weekend Update segment on Saturday Night Live from 1975 to 1976.

He was one of the original cast members of Saturday Night Live, a late night television show which began in 1975.

Soon he became a household name with his infamous catchphrase introduction, ‘I’m Chevy Chase… and you’re not’. He received two Emmy Awards and a Golden Globe Award for his comedy writing and live comic acting on the show.

In late 1976, Mr Chase became the first member of the original cast to leave the show.

His film career soon took off and he featured in titles including Tunnel Vision, Caddyshack, National Lampoon’s Vacation and Three Amigos.

Towards the later part of his career, he appeared in the film Hot Tub Time Machine in 2010 and in 2015, Mr Chase reprised his role as Clark Griswold in the fifth Vacation instalment, titled Vacation.

The Hollywood actor continues to star in films today, however, less frequently than in his youth.

Fans of his work have shared their continued admiration for the veteran actor, one person said: ‘Love Chevy!! His movies were very funny, with good plots yet light and a joy to watch. I am so glad to see him, he looks great!’

Another said: ‘Looks very good for his age.’

‘He was so popular in the 80s.’ a third person commented.

A fourth added: ‘He’s lookin good for almost 80.. fab funny guy!

The When Harry Met Sally actor, who recently recreated a famous outfit from the successful romantic comedy, made his start as a stand-up coming at the age of 16.

But the youngest of three boys born to concert promoter Jack Crystal and Helen Gabler had dreams far away from the showbiz world – he wanted to be a baseball player.

He studied at NYU under Martin Scorsese before embarking on a comedy career, opening for musicians including Barry Manilow.

He got screen time in television shows including as a gay character in the ABC sitcom Soap before becoming a Saturday Night Live regular.

His roles in blockbusters like When Harry Met Sally and City Slickers cemented him as a household name. Crystal’s nine times as Oscar host are the second most in history, behind Bob Hope’s record 19 times between 1940 and 1978.

The comedy legend was last seen on the big screen in 2021’s Here Today, and he also appeared in an episode of Never Have I Ever.

He returned to Broadway in 2022 with Mister Saturday Night, a stage adaptation of his 1992 film of the same name.

Crystal most recently surfaced in a humorous trailer for Jimmy Kimmel hosting the Oscars, which parodied Top Gun: Maverick.

Top Gun: Maverick stars Jon Hamm and Charles Parnell reprised their characters for the trailer, joking that they asked several other stars to host the Oscars before him.

Crystal then appeared as an ‘admiral,’ as Hamm and Parnell asked him to reconsider and host again, though he insisted he had a dentist appointment that night.

‘Listen I’ve hosted this thing nine times and I say give this kid a shot,’ Crystal said, though Kimmel reminded him he hosted twice before.

The beloved funnyman will also return to voice Mike Wasowski in the second season of the animated series Monsters at Work, arriving later this year.

He is married to Janice Crystal and they have two daughters, Jennifer Crystal Foley and Lindsay Crystal.

‘I never realised he was 74. I still think of him as Harry,’ one person said.

One commentator was shocked by the marked change in the actor’s appearance: ‘Why does he suddenly look 100?!’

But others thought he was doing well: ‘He looks great! Would’ve never guessed he’s 75.’

Gene Hackman, 93, began his acting career through a number of summer stock and Off-Broadway plays in New York during the 1950s and 1960s.

He landed his first Broadway role in 1964 as a young suitor in Muriel Resnick’s Any Wednesday where he soon caught the attention of Hollywood agents and was cast in the film Lilith.

By the late 1960s he established his place as a household name and appeared in Arthur Penn’s Bonnie and Clyde in 1967 for which Hackman was nominated for an Academy Award as best supporting actor.

He also featured in films such as The French Connection, which won him the Academy Award for best actor.

Hackman then went on to be cast in The Conversation, Young Frankenstein, and Superman.

Hackman also won the Academy Award for best supporting actor for his portrayal of Little Bill Daggett in Clint Eastwood’s revisionist western Unforgiven in 1992.

His later works include the Runaway Jury in 2003 and Welcome To Mooseport in 2004.

In 2008, while promoting his novel, he confirmed that he had officially retired from acting. Having appeared enjoying his retirement, many were happy to see the actor doing well.

One person said: ‘What a FANTASTIC actor, in so many films. It’s actually terrific to see him, and to see that he’s doing great at 93 – driving, out to a quick lunch on his own, filling up his car himself…’

While another person said: ‘Great age and keeping well. Unforgettable actor.’

A third added: ‘The ageing process is very harsh, but he’s also doing well for 93!’

Tommy Chong, 84, and Cheech Marin, 76, met in Vancouver, in Canada, after Cheech Marin moved there to avoid the draft at the height of the Vietnam war.

Some of their best known works include Earache My Eye and Santa Claus And His Old Lady and their first feature film, Up In Smoke, was released in 1976.

He was absent from the fifth and sixth series of the shows as he was serving time in prison for drug-related offences, becoming the cell-mate of Jordan Belfort, of The Wolf Of Wall Street fame.

Chong was the voice of Yax in Zootopia and appeared on Dancing With The Stars in 2014 before competing in The Masked Singer in 2019.

Cheech Marin, 76, went on to voice many beloved Disney characters, including Ramone from Cars and Banzai the hyena from Lion King. He also featured in the Spy Kids movies, a popular series.

He also won Celebrity Jeopardy in 2010, beating journalist Anderson Cooper.

The comedians reunited in New York earlier this week – but are virtually unrecognisable from their comic duo.

The Iron Man actor started out in film at a very young age – leaving some fans shocked when pictures from November 2022 saw a markedly older Downey Jr.

His actor father first put him in films at five but encouraged a drug addiction that would later derail his entire career.

He would spend more time in correctional facilities and would be fired from Ally McBeal in April 2001 after another series of arrests on suspicion of drug-related offences.

His stepmother, Rosemary, told People Magazine that he had been diagnosed with bipolar disorder, which she believed made his recovery harder.

Downey Jr. got his first post-rehabilitation job lip-syncing for an Elton John video after being unable to join other projects after insurance companies refused to cover him.

Mel Gibson gambled on the actor and paved the way for his comeback, by bankrolling him to appear in The Singing Detective in 2003.

By 2007, his career was back on track and he appeared in the critically acclaimed Zodiac alongside Mark Ruffalo.

He would eventually star alongside Ruffalo in the incredibly successful Marvel films featuring the Avengers, after premiering his Iron Man character Tony Stark in 2008.

A Sherlock Holmes spin-off with Jude Law saw him win a Golden Globe for best actor – a win which he failed to prepare a speech for, as his wife told him Matt Damon would take home the gong.

He retired from the Marvel universe in 2019 after his character was killed off in Avengers Endgame and while a second Sherlock movie was made, the franchise did not continue.

An Alfred Hitchcock remake is said to be in the works for the 57-year-old blockbuster actor and it was announced in March 2023 that Downey Jr. will appear alongside Robert Pattinson in a political comedy from Adam McKay.

Jen Shah is going to be released from prison a year earlier than expected, after she was sentenced to 6.5 years for a telemarketing scheme that preyed on the elderly and vulnerable.

Page Six reports her prison sentence has been reduced by one year, with her release date now set for August 30, 2028.

A rep for Shah, 49, told People in a statement that the Bravolebrity remains ‘committed’ towards making amends with her victims and hopes to return home ‘as soon as possible.’

‘I have spoken with my client Jen Shah over the phone this week — she is doing well and remains committed towards making her victims whole,’ Shah’s rep said.

‘She has initiated her payments towards restitution, and we hope to have her home as soon as possible so that she may resume life with her family and work, putting this chapter of her life behind her.’

Shah, one of the most bombastic characters on the Bravo show, was locked up last month after eventually admitting to a years-long telemarketing scam that preyed on the elderly and vulnerable.

Her surrender to prison on February 17 came after she pleaded guilty to fraud last year – despite previously protesting her innocence for months both on-air and on social media.

She was sentenced to over six years in prison after admitting to orchestrating a telemarketing scheme that targeted susceptible elderly and vulnerable individuals.

Shah duped her victims into buying fake business mentoring, before they found themselves in unrelenting subscription payments they couldn’t get out of.

At her sentencing hearing, prosecutors told how she callously laughed with colleagues when some of the victims called them in tears begging to be released from the debts.

But after being imprisoned for the con, Shah took to Instagram to apologize for her crime.

‘I am surrendering to serve a sentence in federal prison today,’ she wrote.

‘It is the price I must pay for the bad decisions I made. People got hurt because of my decisions.

Earlier this month, Shah’s husband Sharrieff ‘Coach’ Shah read correspondence she had sent from behind bars, where she detailed suffering a panic attack on the way to Bryan Federal Prison Camp, Texas.

‘I could not breathe and my hands went numb,’ she wrote. ‘I knew I was having an anxiety attack.’

Writing about her emotions on her way to prison, the TV personality said she became overwhelmed with emotion while driving to surrender herself alongside her husband.

‘I made Coach pull over because our reception was poor while driving and I was not able to Facetime my oldest baby,’ she continued.

‘We finally connected, I saw his face, and I cried so hard until he reassured me that it would be okay.

‘I didn’t think I’d make it through today. It felt surreal as we drove to Bryan FPC just minutes away from having to surrender.

‘My worst fear and the unimaginable was about to happen – having to say goodbye to my sweet husband and precious baby Omar (he’ll always be my baby even though he’s a senior in HS).’
